Current Families

Each year, we require every current family to fill out a new FACTS Enrollment Packet. This will ensure that we have the most current medication information, authorized pickup list, and emergency contacts.

If your address, phone numbers, email addresses, medical information, authorized pickups, or emergency contacts change throughout the school year, please follow the instructions listed below to update your FACTS Family Portal.

FACTS Family Portal Information Update

Login to your FACTS Family Portal. The District Code is lca-wa.
Under School select Web Forms.
Choose Family Demographic Form.
Click Student Demographic Form to update your student’s cell phone, address, or religious affiliation
Select Student Medical Form to update your student’s doctor's name or phone number, dentist's name or phone number, medical insurance information, permission to treat, medication list, medical conditions, or allergies.
Choose Custodial Parent Form to update a parent’s phone number, email address, home address, religious affiliation, employment information, or parent notification preferences.
Click Emergency Contacts to update who is to be called for emergencies.
Select Transportation Form to update who can pick your child up from school.
Choose Grandparents to update grandparent demographic info.
